Non-detected Stall L6482

Running a bidirectional stepper at 30V with L6482 and getting actual stalls in 16-microstep mode -- under load the motor literally stops and hums, but nothing is reported back to software. It's like the driver has not detected it. We have been getting (apparently spurious) OC and UV errors with various settings, but we seem blind to this actual stall. How can we sense this to troubleshoot or at least halt and recover?
