IDE generates bad USB code

Has anyone had success generating working USB middleware in STMCube32IDE? To date, I've tried generating a VCP or an Audio Class device for the STM32H747-DISCO, Nucleo L452RE-P and Nucleo-L4R5ZI-P. Consistently, all fail after about 30 seconds, with a Windows message of failed enumeration due to a bad Device Descriptor.

I'm using Version: 1.1.0, Build: 4551_20191014-1140 (UTC).

I've also tried generating code using the external CubeMX tool (both on Windows and Mac) and running the code in the IDE. Same failure.
